Preparing for Your Visit
To ensure your appointment goes smoothly, we ask that all new patients complete the necessary paperwork prior to their first visit. This allows Dr. Khan to review your medical history and focus your appointment time on your care.
How to Obtain Patient Forms
Please contact our office to request patient forms or to ask any questions about what to bring to your appointment. Our staff will be happy to assist you with the registration process and provide any forms you need.

What to Bring to Your Appointment
- A valid photo ID (driver's license, passport, or state ID)
- Your insurance card(s)
- A list of all current medications, including dosages
- A list of any allergies to medications
- Any relevant medical records or test results from other providers
- Referral letter from your primary care physician, if required by your insurance
- Your completed patient forms (if provided in advance)

GastroCares accepts most major insurance plans. If you have questions about whether we accept your specific insurance, please call our office at (908) 522-1313 before your appointment. Co-pays and deductibles are due at the time of service.
